The General Data Protection Regulation (GDPR) lays down rules relating to the protection of natural persons with regard to the processing of personal data and rules relating to the free movement of personal data. This includes the right for data subjects to have access to data concerning their health, for example, the data in their medical records containing information such as diagnoses, examination results and assessments by treating physicians and any treatment or interventions provided.
Requests for access to personal data under GDPR should be made in writing and duly signed. In preparing your request, you should follow the following guidelines:

- Identify the records or information that you require.
- Provide full personal contact details.
- Provide a copy of one form of identification, i.e. passport, driving licence or utility bill.
